What to expect:
Your individual needs will be evaluated on a one-on-one basis and will be provided with a comprehensive plan of care & restoration plan based on your body’s restrictions and complexity. Each person is different and therefore has different therapeutic needs. I evaluate and treat the entire body regardless of your complaints or symptoms. This helps provide longer-lasting results by addressing the root cause of the problem, not just treating the symptoms (which only provides limited and temporary relief).
Initial Evaluation: The OT evaluation consists of a new patient questionnaire, brief interview & past medical history review, postural & pelvic evaluation, movement analysis, range of motion testing, manual muscle testing and tissue texture assessment; one-on-one hands-on treatment; and self-treatment exercise instruction (or recovery plan), along with an individualized & comprehensive plan of care.
Follow-up Sessions: Each subsequent session lasts 60 minutes and includes a review or modification to the home exercises and follow-up treatment on residual areas soft tissue restrictions causing pain, abnormal sensations, and compromised function.
Additional self-care therapy tools and exercises may be explored, as well as sustained myofascial stretches targeting affected areas, postural retraining, balance work, strengthening, breath work, & grounding and centering exercises as needed.
For manual therapy sessions, I utilize a combination of Sustained Myofascial Release techniques, including but not limited to structural releases, rebounding & unwinding; craniosacral therapy; soft & deep tissue mobilization; muscle energy techniques; isometrics; energy work; vibration therapy; manual therapy; kinesiotaping, as well as therapeutic use of self, and traditional OT techniques as appropriate.
All rendered services are billed as skilled Occupational Therapy.

I am an out-of-network provider; I do not bill any medical insurance companies directly. Upon request, I can provide you with a superbill to file with your medical insurance company for reimbursement if you wish to file on your own. The super bill includes your name & diagnosis codes (ICD-10); my name, NPI number, tax ID, EIN; the date of service, services rendered (CPT codes billed), and number of units, per visit.
You may be eligible for insurance reimbursement. Please verify your occupational therapy benefits directly with your insurance provider. Insurance plans vary by contract. Medicare and Medicaid recipients are not eligible for reimbursement for out-of-network service providers.
Arizona State is a direct access state. Despite this, some commercial insurance plans will require a script or doctor’s order (Rx) for therapy reimbursement. It is your responsibility to obtain one from your medical provider. I have provided a blank OT Rx form in the RESOURCES tab for your convenience.
Depending on your insurance requirements, your OT referral may be from a physician, osteopathic physician, podiatric physician, surgeon, naturopath, chiropractor, physician assistant, psychologist, optometrist, or advanced registered nurse practitioner licensed to practice in Arizona State. Please contact your insurance provider directly with questions you have regarding your occupational therapy benefits.
